Health forms can be obtained by contacting Mrs. Kelly or through the main office, [email protected]. You may also click the links below to print your own:- State of CT Early Childhood Health Form - required for all entering preschool students. Please click HERE for more information.
- State of CT K-8 Health Form - required before entry to Kindergarten and also prior to the start of Grade 7, but may be submitted annually for student's file in the health office.
- Sports Physical - required annually for basketball and cheerleading.
- Authorization to Administer Medication in School - No medications, not even cough drops, may be taken in school unless this form is signed by your child's physician and the labeled prescribed medication itself to accompany it.
